Submit a Claim - Login

If you are a U.S. resident whose information was accessed in a MGM Data Incident in or around July 2019, and/or in a second separate Data Incident that occurred in or around September 2023, you may submit a claim. Claims must be submitted online or mailed by June 3, 2025. Claim Forms received after the deadline will be deemed untimely and may not be accepted.

If you are an eligible Settlement Class Member, you may submit a claim for one or more of these benefits:

  • Financial Account Monitoring: Use the Claim Form to request one year of three-bureau identity theft protection, Financial Account Monitoring, and at least $1,000,000 of fraud/identity theft insurance.
  • Monetary Reimbursement: Use the Claim Form to request money for one or more of the following:
    1. Reimbursement for Money You Spent. If you incurred financial losses from fraud or identity theft that you believe is fairly traceable to the MGM Data Incident (“Documented Losses”), you can be reimbursed up to $15,000. You must submit documents supporting your claim.
    2. Tiered Cash Payment. Certain Settlement Class Members are also eligible to receive a flat cash payment, without providing documentation, depending on what personal information may have been exposed in the Data Incidents. You are only eligible for this based on the information MGM has provided the administrator.

Please Note: All Settlement Class Members are eligible to file a Claim for Financial Account Monitoring and a Documented Loss Cash Payment, but not every Settlement Class Member is eligible to file a Claim for a Tiered Cash Payment.

Documentation:

If you plan to make a claim for reimbursement of Out-of-Pocket Losses, then documentation must be provided to support your claim(s). Documents should be clear, readable copies, as anything you submit will not be returned to you. Acceptable documentation includes receipts, voided checks, bank statements, or other documents showing the amount of your losses and/or a detailed narrative description.

If you are filing online, please have this information ready before you start to file, as your claim will not be saved if you must come back and finish at a later time. Documents that are uploaded must be less than 20 MB per file and in one of these formats: jpg, jpeg, png, gif, tif, tiff, doc, docx, xls, xlsx, pdf, txt, rtf, or zip. If you are unable to electronically upload a copy of your documents as part of the online Claim Form, you will need to mail a printed Claim Form along with your documentation to the Settlement Administrator.

How to File Online:

To file online, you will need the Unique ID and PIN that is printed on the Notice that you received in the mail or via email.

As part of filling out your Claim Form, if you are requesting reimbursement for Out-of-Pocket Losses, you will be asked to provide information about these losses including the date of loss, the amount and a description of the activities completed due to dealing with the Data Incident. You must also submit all required supporting documentation if applicable. Supporting documentation should be clear, readable copies, as they will not be returned to you.

After submitting your completed claim online, you will receive an email with a confirmation code for your completed submission. Be sure to keep your confirmation email and code and refer to them if you have any questions about your Claim Form.

If you wish to submit a Claim Form via standard mail, you may download a copy of the Claim Form from the Important Documents Page. You will need to provide all the information requested on the Claim Form, sign it, date it, and then mail it to the following address:
